Inspections are broken into Sections, then Items, then Defects and Prompts with Locations. Prompts are not required. Here is an example of the hierarchy for an Inspection with 2 Sections, 2 Items in each Section, 2 Prompts & 2 Defects in each Item, and 2 Locations in each Prompt:
